FIRST TIME ENROLLMENT PAGE 1 Type in form, SAVE AS to your device USING YOUR LAST NAME: Jones_enrollment1.pdf FIRST TIME ENROLLMENT PAGE 2 Type in form, SAVE AS to your device USING YOUR LAST NAME: Jones_enrollment2.pdf
Once these are submitted, administrator will invoice you and email you the first year enrollment forms and an email explaining more about attendance, etc.
